Help Kevin get on the ballot!

One of the easiest ways to help Kevin's Campaign for State Senate is to assist us in obtaining signatures from the required 1% of registered voters in Senate District 12 (about 4000 verified signatures) to allow Kevin's name to be officially placed on the ballot

Please help Kevin qualify to be on the ballot as a candidate for the Florida Senate District 12 seat, by signing the Candidate Petition Form below and mailing the original to our Campaign Headquarters. We would appreciate your help in spreading the word to your friends and neighbors too.

INSTRUCTIONS:

Please print out the Candidate Petition Form using the Printer-Friendly PDF Button and fill in the information listed below.

(You can be a registered voter with any party (Republican, Democrat, Independent, etc.) to fill out the form as long as you live in Senate District 12. Signing the Candidate Petition does not require you to vote for the candidate.

4. Your Date of Birth or Your Voter Registration Number (you only need to fill in one or the other, but not both)
